  The UI Payroll Office will deduct regular-pay faculty members’ dues in 20 installments per year, from September through May. Please check the appropriate line for your per-pay dues deduction level.

  Payroll Deduction Authorization*

  I hereby authorize the UI Payroll Office to deduct this amount from each of 20 pay periods per year for the UI Federation of Teachers. I further authorize the UI Payroll Office to automatically align my deduction level to correspond to my current salary. This authorization will remain in effect until I make a written request to stop the payment.
